As providers of community-based hospice services since 1977, Hospice Services of St. Joseph Health provides individualized, compassionate care to patients and families, as well as grief support and end of life care education to the community. Our ongoing commitment to you includes:

  • Honoring the dignity of individuals who are preparing for the end of life.
  • Providing patient centered care which focuses on physical and emotional comfort.
  • Working with a team that includes Nurses, Hospice Physicians, Social Workers, Spiritual Care Providers, Hospice Aides and Volunteers.
  • Supporting grief service needs for adults, teens and children in both English and Spanish. We provide one-to-one counseling, groups, volunteer support and grief education classes.
  • Providing consultation services through Hospice Physicians.
  • Delivering services regardless of the ability to pay.

Hospice Services are designed for someone with a life limiting illness, whose eligibility for hospice care is certified by their doctor. Patients who elect hospice care have an illness for which treatments are no longer beneficial, or who have decided that the treatments are not working and are too burdensome to continue. Our focus at Hospice Services of St. Joseph Health is to offer comfort, peace, and dignity for our patients and their families through expert patient care and support services.

Our team of compassionate professionals will assist in caring for the patient and their loved ones in their own home, a skilled nursing facility, assisted living community or wherever they reside.

In addition to providing specialized medical care to the patient, hospice also provides emotional and spiritual support to the patient and his/her loved ones, as they go through what is often a challenging time. All services are individualized to the personal needs of the patient and family.

Cost of hospice

Families are relieved to know that the Medicare Hospice Benefit pays 100% for care, and that hospice is also paid for by Medi-Cal, the VA, and private insurance. No one is denied care because of an inability to pay. Using generous donations from the community, fund raising events, and the profits of our two thrift stores, we are able to provide services at no charge.

Our hospice services include
Hospice Services of St. Joseph Health view grief as a normal process that can be accompanied by fear, loneliness and uncertainty. Our Grief Support Services are designed to provide emotional and social support and education to individuals through one-to-one counseling, support groups, events and workshops.

Our Grief Support Services are unique in that we provide grief support to both hospice family members and to anyone in the community, whether or not they have used our hospice services.

Grief services are supported by donations and fund raising efforts, and are offered in both English and Spanish, at no charge to the recipient, although any and all financial contributions are gratefully accepted.

Each year our hospice programs provide services to families in need of care and support.

Donations and in-kind gifts to Hospice Services of St. Joseph Health help support hospice care for community members who are unable to pay. They also support grief services and community education projects, as well as volunteer training and other programs the benefit Sonoma and Marin Counties.

As a service of St. Joseph Health Hospice Services our programs raise necessary funds through events and donations for community services.

These funds support compassionate, comprehensive medical hospice care for under insured patients nearing the end of life; spiritual and social services support for patients and family members; grief support services for children, teens and adults; training for caregiver and grief support volunteers, community education to provide awareness about end of life questions and concerns; and Latino patient care and grief support services.

Upcoming classes and events

Over 550,000 people volunteer at hospices around the country, donating 25 million hours of service. Volunteers are an integral part of our hospice team. Our volunteers receive extensive training and ongoing education and support.

There are plenty of ways for you to share your gifts with our patients, families, community and staff. There are opportunities to volunteer in the home and extended care settings as well as in the office providing support to the Hospice Team. In addition, we have need for volunteers in grief support and at special events.

Hospice volunteers are interviewed and screened. Then, each volunteer participates in a comprehensive 8 week training which helps to prepare them to be an important part of our Hospice Team.
